Certified Nurse Assistant

Job Summary

Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family, and share observations and issues with the supervisor. Works under the direction and supervision of a Registered Nurse. Functions in a supportive role to all IDT members.

Job Qualifications

  • Education: High School diploma Preferred.       
     
  • Licensure: Must be certified by a State licensure program, and currently listed in good standing on the State nurse aide registry.  Must have a current and valid Texas State Drivers License.
     
  • Experience: At least one year experience in the last 24 months as a Home Health/Hospice Aide or Certified Nursing Assistant in a hospital, nursing home or home health/hospice agency, or completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
     
  • Skills: Must be able to read and write in English, and follow written and verbal instructions in English effectively. Demonstrates interest in the welfare of ill and elderly. Proof of current Hepatitis consent/declination and TB vaccination record. Complete Aide Competency Evaluation Skills Checklist and written Skills Test. Ability to use electronic medical charting software.
     
  • Transportation: Reliable transportation. Valid and current auto liability insurance.       

Essential Functions

  • Performsimple procedures as an extension of nursing services.
  • Provide services that are permitted to be performed under State law by hospice aides.
  • Provide services that are consistent with the Hospice Aide training.
  • Complete appropriate records in compliance with hospice policies and procedures.
  • Promote positive, supportive, respectful communication to patient/family and other employees.
  • Provide skills necessary to assist the patient with proper nutrition, adequate fluid intake, and personal hygiene.
  • Contribute to the management and efficient operation of the company, and demonstrate effective time management skills.
  • Provide skills necessary to perform procedures according to company policy.
  • Demonstrate commitment, professional growth, and competency by attending a minimum of 12 hours of annual in-services.
  • Complete an annual supervised session of skills competency evaluation.
  • Provide an environment which promotes respect for patient, privacy, and property.
  • Hospice Aides must report changes in the patient’s medical, nursing, rehabilitative, and social needs to a Registered Nurse, as the changes relate to the plan of care and quality assessment and improvement activities.
  • Promote the company philosophy and administrative policies to ensure quality of care.
  • Provide direct patient care to patient under direction of the Registered Nurse and according to the Aide Plan of Care.
  • Assist with exercises when included in plan of care.
  • Provide reminders and assist in medications that are ordinarily self-administered.
  • Provide necessary skills to assist the patient with safe transfers and ambulation per company policy.
  • Provide necessary skill to appropriately report changes, and document pertinent information and care rendered to patient to ensure continuity of care.
  • Practice accepted infection control principles.
  • Provide a clean, safe and comfortable environment.
  • Perform other duties/supportive tasks as specified
