Job Summary
Primary function is to coordinate home health care under the supervision of a registered nurse, for clients of all ages in their primary place of residence with the interdisciplinary team, patient/family and referring agency/source.
Job Qualifications
- Education: Graduate of an accredited School of Vocational Nursing.
- Licensure: Current Texas State license as a Licensed Vocational Nurse, and a valid Texas Drivers license.
- Experience: Two years experience as a Licensed Vocational Nurse in a clinical care setting, home health preferred.
- Skills: Nursing skills as defined a generally accepted standards of practice. Good interpersonal skills. Proof of current CPR, and Hepatitis profile.
- Transportation: Reliable transportation. Current auto liability insurance coverage.
Essential Functions
- Under the direction of the RN, assists in identifying the patient’s physical, psycho-social and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.
- Participates in planning, coordinating, and implementing care in conjunction with the Interdisciplinary team in accordance with the POC.
- Provide care utilizing infection control measures that protect both staff and patient (OSHA).
- Assure the continuity of care through scheduling patient care.
- Provide effective communication to patient/family team members, and other health care professionals as evidenced by documentation, clinical records, case conferences, communication notes, and evaluations.
- Monitor cases to ensure compliance with requirements of third party payor.
- Performs others duties as assigned or requested.
- Promotes company philosophy, mission, and administrative policies to ensure quality of care.
- Uses effective interpersonal relations and communication skills.
- Demonstrates commitment, professional growth, and competency.
