Community Based Alternatives

High-Quality, Compassionate, Accessible Home Health and Hospice CareThe Community-Based Alternative (CBA) Program provides home community-based medical services to patients age 21 and older. These eligible services are provided as cost-effective alternatives to placement in nursing facilities and allow the patients a higher degree of independence in surroundings that are familiar to their care.

Case Management is provided by the assigned caseworker at the Texas Department of Human Services (TDHS). This waiver service includes:

  • Skilled Nursing Care
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Respite Care
  • Personal Assistance Services
  • Minor Home Modifications
  • Adaptive Aids
  • Medical Supplies

Qualifications

Participants in the CBA program must be 21 years of age or older. They must be Medicaid eligible in the community under SSI, MAP protected status or meet the income and resource requirements for Medicaid benefits in nursing facilities.
