Patients and Families

If you are a patient in need of service, or if you are currently on service, please call the IntegraCare office that is closest to you. Click here to view a list of locations. Providers

If you are a physician looking to provide a referral, please visit our unique Provider Portal. If you are among any of the other professional provider organizations, please submit a referral through our online inquiry form. Alternatively, you may provide a referral via phone directly to one of our branch offices.
