Primary Home Care

High-Quality, Compassionate, Accessible Home Health and Hospice CarePrimary Home Care is a non-medical personal care service prescribed by a physician as part of a patient's plan of care. IntegraCare can provide primary home care to eligible Medicaid clients whose health problems cause them to be functionally limited in performing the activities of daily living. Patient services are provided by a supervised Personal Care Attendant, who performs tasks based on individual need.

The Personal Care Attendant has been trained by a highly experienced, licensed nurse to perform the following duties:

  • Changing bed linens
  • Light housecleaning
  • Dressing and laundry
  • Shopping and escort assistance
  • Meal preparation, feeding and dish washing
  • Exercising
  • Assist with self-administered medication
  • Toileting
  • Transferring/ambulation
  • Routine hair and skin care

Qualifications

  • Receive Medicaid or meet the following requirements:
    • Individual: Monthly income of $1,500/month; Resources $3,000/month
    • Couple: Monthly income of $2,000/month; Resources $3,000/month
  • Have a doctor's order for Primary Home Care
  • Have a functional disability
  • Need at least one personal care task
  • Require assistance with activities of daily living
